  • How do I send a referral?
    Call (801) 375-1414 and the front desk will coordinate scheduling and records with your office. Please do not send patient health information through unsecured email.
  • Do you bill medical or dental insurance?
    Sleep and airway treatment is typically billed to medical insurance. TMJ care is mixed. We review each case individually and help patients understand their coverage before treatment begins.
  • Will I get a plan summary?
    Yes, with the patient's authorization: a written summary after the evaluation and updates at significant treatment milestones.
  • Which patients should I refer?
    Snoring or diagnosed sleep apnea patients who are candidates for oral appliance therapy, CPAP-intolerant patients, children with airway concerns, and patients with TMJ pain, facial pain, or headaches with a suspected dental component.
  • Can patients self-refer?
    Yes. No referral is required to be seen. When a provider refers, we coordinate records and updates with the referring office.
